Irma van den hadelkamp

My name is Irma van den Hadelkamp.  I am a Registered Nurse working for the Peaks to Prairies PCN Women & Infant Health program in Sundre since April 2014.  My job at the clinic is to prepare and educate all maternities during their pregnancy, labor and transition to motherhood. On your prenatal visits you will visit me and your family doctor. We like to work as a team together with you, so we can give you a good pre and postnatal experience.  I do follow up appointments until the baby is 6 weeks old, but you are always welcome to book an appointment with me if you need any advice after this period.  Since October 2017, I am an International Lactation Consultant and I am happy to assist you when you experience any breastfeeding challenges.

Donna HOPTON

I have been a Registered Nurse for over 25 years and received my training through Humber College in Ontario.  After graduating  I worked in Sparta Tennessee  for a short time before returning home to Alberta where I was hired by Sundre General Hospital.  In 2003 I began working as an RN for Greenwood Family Physicians until 2014 when I was hired as a Case Coordinator by Peaks to Prairies Primary Care Network.   It was perfect for me, I was able to remain at Greenwood Family Physicians with the Staff and Patients I had grown very close to!  I continue working within Greenwood Family Physicians Clinic, in the field of Chronic Disease Management and am available to Patients 3 days per week interested in making healthy lifestyle changes to manage Diabetes,  hypertension, weight management, cholesterol or smoking cessation.

As a Clinical Exercise Physiologist, I specialize in the prescription of exercise and wellness interventions for chronic disease management, disability support, and weight management. I lead evidence-based programs such as GLA:D Canada, GLA:D Back, Breathe Easy, and BoneFit for Osteoporosis, in addition to providing myofascial release treatments to promote functional movement and recovery.
I have worked with individuals across a broad range of life stages and health conditions, helping them access and engage in physical activity in a way that is safe, effective, and enjoyable. My focus is on empowering individuals with the education and support they need to live an active and healthy life, regardless of their starting point. Outside of my professional role, I am an avid outdoor enthusiast who values quality time with my family. We engage in activities such as camping, fishing, biking, hiking, swimming, skiing, and a variety of other outdoor adventures. I am proud to be part of the Peaks to Prairies PCN team and look forward to continued collaboration with healthcare professionals to support patients in achieving their health and wellness goals.

Cameron Rice

Hi, I’m Cameron. I’m a registered social worker and mental health counsellor at Peaks to Prairies PCN. After five years of study and frontline experience, I recently completed my Bachelor of Social Work and stepped into this role with deep gratitude and excitement.

 

My approach combines mindfulness, metacognitive practices, and a strengths-based perspective to support individuals seeking mental health assistance and guidance in navigating various systems. I am particularly passionate about creating a space for men to explore vulnerability and healing, and I strive to reduce the stigma surrounding help-seeking, especially in rural communities.

 

Outside of work, you might find me spending time with my growing family, running a Dungeons & Dragons campaign, or listening to loud music that reminds me of my teenage punk years, filled with big feelings and even bigger hair. I believe that healing is possible for everyone and that the best work occurs when we feel seen, safe, and supported.

ALETHEIA SOMERVILLE

My name is Aletheia Somerville and I am a registered nurse. I received my Bachelor’s Degree in Nursing in 2008 and have primarily worked in intensive care in both the US and Canada. My background and experiences working with patients and their families in the ICU have inspired me to join the PCN and be part of the support network keeping our seniors healthy. It is a privilege for me to work with our seniors as they have an important role in our communities and lives. I live in rural Mountain View County with my family and we love the beauty of our surroundings and being part our rural community!

Marjie Goertzen

My name is Marjie and I have been a registered social worker since 2007. I am currently pursuing the Master’s of Social Work at Dalhousie University. My background includes 15 years in school social work, supporting families with complex needs, navigating the system, providing prevention, crisis intervention, and early intervention. My experience also includes Alberta Health Services, in mental health (inpatient and outpatient), as well as medical social work. I participated in an internship with the Children’s Grief Centre in Calgary and have been privileged to support people walking through grief and loss.

 

I feel very blessed to be working at the PCN, walking alongside, and supporting people to improve their well-being and participate in their journey of healing. I have a heart for supporting individuals of all ages and families experiencing the challenges of complex diagnoses and health challenges. Part of my focus at the PCN will be on perinatal mental health. I feel fortunate to be able to assist during this critical phase of people’s lives, supporting emotional well-being with empathy, empowerment, advocacy, and trauma-informed care as they navigate the challenges and joys of parenthood. I am currently pursuing a certification in Perinatal Mental Health.

 

I have always lived and worked rurally. I grew up on a farm near Didsbury and currently reside in rural Mountain View County on a small hobby farm with my family. I enjoy spending time with our menagerie of animals, from goats to an alpaca. Animals have always been a source of comfort and healing in my life and I feel grateful that my children can experience the joys of living close to nature.
